Request for Certificate of Insurance Application

All NWBA registered members, individual/team/affiliate, are eligible for insurance coverage. Insurance coverage is necessary for practice location, fundraising events and competition (games/tournaments). Although request can typically be turned around within a few days, please allow 5-7 business days for processing.

NWBA Expense Voucher

Reimbursements are available for approved expenses associated with NWBA operations.  If funds are to be reimbursed for conference or divisional expenses this report must be approved by the conference or divisional leadership.

After approval, please forward the voucher and supporting documents/receipts to Anthony Bartkowski at anthonybartkowski@nwba.org.

NWBA Hall of Fame Nomination Application

Nominations for membership to the Hall of Fame may be made by any member of the NWBA and submitted to the NWBA Hall of Fame Committee for its consideration via the official nomination application. Nominations must be submitted by February 24th of each year to the Hall of Fame Chairperson. A citation shall then be drawn up for each worthy candidate by the Hall of Fame Committee and presented to the NWBA Board of Directors for its final determination.

Wayne Kunishige Memorial Scholarship - Application

The National Wheelchair Basketball Association is pleased to annually present the Wayne Kunishige Memorial Scholarship. The scholarship will be presented to a NWBA player who is a high school senior and planning to attend college or other educational endeavors.

The Wayne Scholarship honors Wayne Kunishige, a long time NWBA member, who died in December of 2000.  When Wayne was 15, he sustained transverse myelitis while playing in a church league basketball game. He fell on the court and never walked again, living his life with a spinal cord disability. He never lost his love for the game and while in high school and college in Fresno, CA he played for an adult team, the Valley Freewheelers as there were no Junior programs at that time. He later started the Fresno Red Rollers, and achieved a lifelong dream when his team won the national championship at the 47th NWBT in 1995.  Wayne graduated from California State University, Fresno and worked in the health care field his entire career. He spent nearly 20 years working for Sunrise Medical Quickie Wheelchairs, where he was Vice President of Advanced Development and one of the innovators of the Quickie line of sports wheelchairs.

Wayne’s family and friends endowed this scholarship in his memory, in hopes that it would help another young wheelchair basketball player get a start in life. Sunrise Medical Quickie honors Wayne’s accomplishments with the contribution of a Quickie All Court basketball wheelchair.

2018 Duncan Good Guy Award

The NWBA is pleased to present the annual  Bill and Julie Duncan Good Guy Award.  Established to recognize a NWBA volunteer who exemplifies a commitment to wheelchair basketball at the grassroots level through contributions to a local team, conference or the national organization, the award is named for  Bill and Julie Duncan who dedicated decades of service to the NWBA.

The award will be presented at the 2018 National Wheelchair Basketball Tournament presented by ABC Medical in Louisville, KY. Applicants do not need to be present to be selected for the award. The award includes a commemorative plaque and a $350 award to the team of the recipient’s choice.
